Please be sure to follow through the next three screens until you get to the button to submit your selections to the Rectory. Your choices will not be saved until you submit them at the end. Thanks.

Step 1: Family Information

Complete Family Information Boxes – Family Name, Address and Phone must be included. Each individual in a family can submit their own form or all names in a family can be submitted on one form. (This information will not be used for any purpose other than to submit your commitments.)

Next complete the PIN table – enter the name of each person in your family next to a PIN number (e.g. #1 Johnny, #2 Suzie, #3 Mikey)
